Survey of Alternative Transportation
For The Vision-Impaired

MD Support is asking for your help in gathering information about transportation services in your area. Your input is greatly appreciated, and participation is completely anonymous. Your name will not be added to any mailing list for promotional purposes.

Purpose:

The Americans With Disabilities Act of 1990, Title II, requires that acceptable alternative transportation be available in every area of the country for people who are significantly vision-impaired. MD Support is creating and maintaining a national database which identifies transportation services and the accommodations they provide.

The searchable database may be accessed from the pull-down menu above or by selecting "Transportation Search" in the links to the left.

Instructions:

The button at the bottom of this page will take you to the easy-to-use survey form. There you will be asked to:

1. Select the types of transportation available in your area. (Preview the transportation options.)

2. Provide as much contact information as you can for those services. This includes the name of the service, street address, phone, email, and web site. Important: Enter only services that offer accommodations for the vision-impaired. Other services, however, may be included if no such alternative transportation is available. Please do not enter travel agencies, hotel shuttles, or rental companies.

3. Check off the types of accommodations offered for the vision-impaired. (Preview the expected accommodations.)

4. When finished entering information for each service, click "Submit."

Please be as complete and accurate as possible, and remember that any good information is better than none at all. Each time you click the "Submit" button, your information is saved in our database. This means that you are free to leave and return later to make additions or changes.
